Limit one entry from this form per person. Only completed forms submitted online and received before 12 midnight, CT, on January 31, 2022, will be eligible. Three (3) winners of “MY Petfood Forum” stories will be selected by, and at the sole discretion of, the Petfood Industry and Petfood Forum teams from all eligible entries and notified on or before February 15, 2022. By entering, entrants agree to the potential use of their name and story by Petfood Industry and Petfood Forum in promotional messages. Stories may be edited by our editorial teams before being published.
One (1) winner will receive one (1) $100 Amazon gift card. Two (2) winners will each receive one (1) $50 Amazon gift card.
